  • Mailing of Claim Forms

  • Submit Claim Form

As outlined in the Claims Protocol, which is attached as Exhibit A to the Plan of Allocation, Class Members must submit a completed Claim Form to the Cal-Shake Administrator at:

  • Assignment Form

If you are in need of an assignment form to assign your claim to a seller or purchaser of a property, please click here.

  • Roof Inspection

If the Settlement Administrator determines that your completed Claim Form satisfies the criteria set forth in the Plan of Allocation, you will be notified that your claim is considered eligible and that it will be forwarded to the Chief Roofing Inspector to be scheduled for an inspection. Due to the number of claims filed, it may take longer than 90 days from receipt of the claim by the Chief Roofing Inspector before the property can be scheduled for the roof inspection.

  • Reimbursement

After a Class Member has received a roof inspection from a Cal-Shake Inspector, a report will be submitted to the Administrator by the Inspector, stating whether the property is eligible for payment. If the Claim is eligible, the Administrator will pay the Class Member pursuant to the Plan of Allocation.

If the Cal-Shake shakes on your roof were manufactured between January 25, 1980 and January 31, 1986, this current settlement will not cover your claim. The settlement period for Old Cal-Shake shakes has ended.
